We present the case of a patient who underwent varicose vein surgery on both legs twenty years ago. Over time, the varicose veins reappeared, causing edema, pigmentation, dryness, new varicose clusters, and, in addition, osteoarthritis in the knees.

In the surgery performed two decades ago, Doppler ultrasound was not used as part of the evaluation. At our center, however, the first step was to perform this study, a fundamental tool to identify the true origin of varicose veins. Thanks to this assessment, it was determined that the patient had insufficiency in the saphenous trunks, which were not treated in the previous surgery. This explains the recurrence of the disease.
The patient herself reported that, after the first surgery, the varicose veins disappeared temporarily but soon reappeared. The cause was the absence of an adequate initial Doppler evaluation, which would have allowed proper treatment of the root problem: venous reflux in the saphenous trunks of both legs.
